Website Terms & Disclaimers
Educational purposes only. Content on this site and in our free resources is for general education. It isn't medical advice or a substitute for an individualized evaluation.
No therapist-client relationship. Visiting this site, joining our email list, or submitting a form doesn't create a therapist-client relationship. That begins only after an evaluation and a signed enrollment agreement.
No guarantees. Every child is different, and outcomes vary. We can't guarantee specific results.
Not for emergencies. If you believe your child is experiencing a medical emergency, call 911 or seek emergency care.
ESA funding. Foundations First Co. is [an approved provider for ESA direct pay]. Eligibility, award amounts, and funding approvals are determined by the state ESA program, not by us, and families are responsible for confirming their available funds.

Placement: Make two pages in Squarespace, "Privacy Policy" and "Terms & Disclaimers," and link both in your site footer. Put one line right under your signup form: "By signing up, you agree to receive emails from Foundations First Co. Unsubscribe anytime. See our Privacy Policy." Squarespace also has a cookie banner option in your site settings, which is worth turning on.
Two things to check before you rely on these:
Health information. Your later intake form will ask about your child's concerns, which is health-related. A plain Google Form or email isn't a secure place for that. Look into whether HIPAA applies to your practice (it depends on how you bill) and ask your liability insurer or the Arizona OT board what they expect, then use a secure intake platform when you get there. Keeping the signup to name and email, as planned, avoids the issue for now.
Advertising rules. Check whether the Arizona OT board has rules on how you display credentials or describe yourself in ads, including words like "specialist."
